RECORDS & REPORTS REQUESTS:

Questions concerning report or citation information can be obtained during general business hours. Hours of operation are Monday through Friday 8:00 am to 4:30 pm. The office telephone number is 847-438-9050.

For copies of traffic crash reports, submit a written request and send it to the Police Department Records Office at 2 Lagoon Drive., Hawthorn Woods, IL 60047. Enclose a $5 check or money order payable to the Village of Hawthorn Woods. Please provide a report number if available, or note the date time and location of the incident. Don't forget your return address. You may also obtain copies of crash reports during normal business hours at the Police Department.

To obtain other police reports a Records Request Form must be completed pursuant to the IL Freedom of Information Act and returned to the Police Department.
